Chronic benign neutropenia of infancy associated with asymptomatic cytomegalovirus infection

Abstract

A 7-month-old infant with chronic benign neutropenia and hyperlymphocytosis is reported. The neutrophil count ranged between 320 and 810/mm3 during the first 6 weeks of study. The lymphocyte subpopulations showed a relative increase in the B cell count (33%) and a low T4/T8 ratio (0.78). Leukoagglutinins were not demonstrated. He was found to have an asymptomatic cytomegaloviral infection by culturing the virus from his urine. Before a surgical procedure, he was treated with endovenous immunoglublin showing a temporary reversal of neutropenia (up to 10 000 neutrophils/mm3) as well as an increase in the T4/T8 ratio (1.50). Because of the therapeutic response to the immunoglobulin, an immunopathogenia mechanism for neutropenia is proposed, being cytomegalovirus the possible etiologic agent.
