Abstract
All treatment modalities aimed at prevention of a spontaneous preterm delivery - with the exception of a prophylactic cerclage in case of an evident insufficiency of the cervix - were till recently ineffective. However, prophylactic administration of progesterone by weekly injections or by daily vaginal administration from the 16th week of gestation onwards reduced significantly the incidence of spontaneous preterm delivery in pregnancies at high risk for preterm delivery as recently described in two randomised studies. After a spontaneous preterm delivery couples need to be informed about this prophylactic treatment in a next pregnancy.
